A pistol powder vs. rifle powder is a pretty big oopsie. Powder containers are clearly marked, and if there is suspect powder in the measure, a prudent reloader would simply discard it and refill with a known quantity. The only time I've seen something like this happen was when somebody decided to start reloading and did no reading/classes/anything. They filled up a rifle cartridge with said pistol powder, and the rifle came apart on the first shot.
